Registered Nurse - Float

JOB DESCRIPTION SUMMARY

Responsible for the overall management of all nursing care for assigned patients. Under

the direction of the Hospice Medical Director, plans and provides individualized skilled nursing care to enable patients to participate as fully as possible in the activities of daily living, in both in-home and in the in-patient settings.

When assigned to homecare, the Float RN will provide prompt response to physical and emotional needs of the patients and their families in the form of phone calls and/or home visits.

The Float RN may also be utilized as a fill in for staff nurse coverage at Stoneleigh Residence (our Hospice House in-patient unit).

This nurse must proficiently problem solve case management, symptom management and crisis situations that arise. The float nurse participates as a member of the Interdisciplinary Team (IDT) to plan and provide holistic care to patients and families, as needed. Nursing care reflects observations, actions and plans that are in accordance with the IDT Plan of Care.

The successful candidate should possess:

• Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care.
• Current licensure in State and CPR certification.
• Bachelor’s degree, with one (1) year of hospice care experience preferred.
• Management experience not required. Responsible for supervising hospice aides.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Salary: $35-38/hour

Expected Hours: 30-40/week
