Licensed Practical Nurse (LPN)
We are seeking a compassionate, dedicated, and bilingual (Arabic/English) Licensed Practical Nurse (LPN) to join our Value-Based Care team. This role is specifically focused on supporting patients enrolled in our Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM) programs at our Medical Center. The ideal candidate is a proactive, patient-centered clinician who thrives in a care coordination environment and is committed to improving outcomes for our chronically ill patient population. Key Responsibilities:
• Provide direct patient care and ongoing care coordination for patients enrolled in CCM, PCM, and RPM programs
• Monitor, assess, and document patient conditions, vital signs, and remote health data transmitted through RPM devices
• Conduct regular patient outreach and follow-up calls (in English and Arabic) to ensure adherence to care plans
• Collaborate closely with physicians, care managers, and the broader healthcare team to develop and update individualized care plans
• Educate patients and families — in both English and Arabic — on chronic disease management, medication adherence, and lifestyle modifications
• Track and document monthly care management activities in compliance with CMS guidelines for CCM and PCM billing
• Identify and escalate changes in patient condition or emerging care gaps in a timely manner
• Ensure compliance with all healthcare regulations, HIPAA standards, and value-based care program requirements
• Maintain accurate, timely, and thorough documentation in the Electronic Health Record (EHR) Qualifications:
• Current, valid LPN license in the State of Michigan (required)
• Arabic/English bilingual proficiency — written and spoken (required)
• CPR/BLS certification (required)
• Experience or familiarity with CCM, PCM, or RPM programs strongly preferred
• Strong understanding of chronic disease management (diabetes, hypertension, COPD, heart failure, etc.)
• Proficiency with EHR systems and remote monitoring platforms
• Excellent communication, empathy, and patient engagement skills
• Ability to work independently, manage a patient panel, and meet program compliance targets
• Prior experience in a value-based care, care management, or outpatient/clinic setting preferred Benefits:
