Hospice Certified Nursing Assistant - Livingston County

Schedule: Monday–Friday | Day Shift

Position Type: Full-Time | Field-Based

At Elara Caring , we bring hospice care to where patients feel most comfortable — their own homes. We’re looking for a Hospice Certified Nursing Assistant (CNA) who is passionate about providing personal care, comfort, and support to patients and families during life’s most meaningful moments.

Key Responsibilities

• Provide hands-on care to hospice patients in their homes

• Assist with bathing, grooming, and daily living activities

• Offer emotional and physical comfort to patients and families

• Report changes in condition to the RN Case Manager

• Document all care provided accurately and on time

• Support patient safety and help reduce hospitalizations

Qualifications

• High School Diploma or GED required

• Current CNA certification and completion of a state-approved Hospice Aide training program (per 42 CFR §484.36)

• Minimum 1 year of experience as a CNA or Hospice Aide (hospice or home health preferred)

• Reliable transportation, valid driver’s license, and auto insurance

• Ability to lift 50–100 lbs and perform hands-on patient care

• Residence within or near Livingston County preferred
