Pro Care by Abound Health is seeking a Care Coordinator to join our team in Southfield, MI.
You’ll lead client care coordination, support families and staff, and ensure high-quality service delivery for individuals receiving Michigan Waiver services. This is a hands-on, impactful role where you’ll drive outcomes and make a real difference in people’s lives.
Key Responsibilities:- Manage assigned client cases, ensuring services are delivered accurately and on time
- Review and maintain all client and staff documentation to meet regulatory standards
- Conduct client and staff meetings to ensure compliance with care plans and policies
- Develop and track individualized goals, interventions, and outcomes
- Coordinate staffing schedules and ensure proper coverage for all services
- Serve as the main point of contact for clients, families, and community partners
- Resolve concerns, document incidents, and ensure quality care delivery across all cases
- Experience in care coordination, case management, or related healthcare/social services field
- Strong organizational skills and ability to manage multiple priorities in a fast-paced environment
- Excellent communication and relationship-building skills
- Detail-oriented with strong critical thinking and problem-solving abilities
- Ability to work collaboratively with clients, families, and interdisciplinary teams
- Proficiency in Microsoft Office and documentation systems
- High School Diploma/GED
Base pay is determined by factors such as location, experience, skills, and education. In addition to base pay, we offer a comprehensive benefits package including medical, dental, and wellness benefits starting first of the month following 60 days.
