Key Responsibilities

• Conduct behavioral and developmental health screenings and assessments.
• Support patients and families in managing diagnoses and accessing resources.
• Provide brief interventions to assist with behavioral and lifestyle changes.
• Collaborate with healthcare providers to develop patient-centered care plans.
• Maintain accurate and timely documentation for all behavioral health services.
• Advocate for patients and connect them with relevant community services.

What We're Looking For

• Education & Licensure:
  • Master's degree in social work (MSW)
  • Active LMSW license (Michigan)
• Experience:
  • 3 + years in social work (preferably in primary care, outpatient, or hospital settings)
• Certifications:
  • CPR Certification (American Red Cross or AHA)
  • Perinatal Mental Heatlh Certification (or willingness to obtain within 6 months of hire)
• Skills:
  • Strong patient advocacy, case management, and collaboration abilities
