CarePlus TLC is seeking a dedicated, compassionate, CNA/HHA (Home Health Aid) with a passion for serving others to join our team!

This role requires a commitment to delivering high-quality care while maintaining a clean and safe environment for clients. The successful candidate is self-motivated, reliable, compassionate, and committed to creating a positive experience in our client’s lives

• · Available shifts are Tuesdays and Thursdays 4.5 hours and occasionally on Saturdays

As a Home Health Aide/CNA at Care Plus TLC , you will play a vital role in assisting patients with daily living activities and providing essential health care support. This position is crucial in enhancing the quality of life for our clients while promoting their independence.

*Must have a valid driver's license

*Must own your own reliable vehicle

Responsibilities:

• Knowledge and experience of dementia care is essential
• Help with mobility and transportation needs for appointments or errands.
• Assist client with monitoring Blood sugars and adhering to appropriate low sugar diet
• Monitor patient’s condition and report changes/concerns to appropriate medical professionals and family members
• Provide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, grooming, etc.
• Perform light housekeeping duties to maintain a safe and clean-living space for clients.
• Provide companionship and emotional support to enhance patient well-being.
• Understands First Aid and Emergency Care protocols
• Complete electronic documentation while on shift
• Medication reminders
• Adhere to HIPAA regulations and maintain patient confidentiality.

Skills:

• Ability to work effectively with individuals with developmental disabilities or dementia is desirable.
• Familiarity with Home Health Care, safety and sanitation standards and procedures
• Understanding of Confidentiality and Privacy Laws
• Excellent communication and interpersonal skills… able to communicate effectively with patients and their families.
• Knowledge of basic medical terminology
• Detailed oriented
• Ability to undertake a variety of diverse care tasks and work as a team member
• Compassionate and able to handle tough situations as needed
• 1 year experience as a caregiver is preferred.
