Minimum Requirements

• Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Michigan Physical Therapy license required to start the assignment.
• BLS (Basic Life Support) certification from AHA or ARC may be required.
• Experience in skilled nursing or long-term care settings preferred.
• Experience providing restorative nursing therapy and skilled PT services, including gait training, therapeutic exercises, transfer training, and use of standard SNF equipment preferred.
• Ability to complete documentation according to facility and regulatory standards.

Assignment Details

• Facility Type: Skilled Nursing Facility.
• Shift Name: Days.
• Shift Details: Days 08:30–16:30.
• Typical workweek: 5 days per week, 8-hour shifts.
• Assignment Length: 13 weeks.
• Guaranteed hours: 35 hours per week.
• Candidates must be able to work in a long-term care environment and may be expected to coordinate care with interdisciplinary teams.
