Hospice Clinical Team Assistant (Administrative)

As a Team Assistant at Emmanuel Hospice, you will experience a rewarding work life in a full time, hourly position.
Responsibilities
- Coordination of team needs through daily communication with the clinical team.
- Attends Interdisciplinary Team Meetings, acting as scribe and managing IDT attendance, meeting invitation, and the scribe rotation schedule.
- Enters and updates information in the medical record as assigned.
- Performs visit verification activities weekly. Verifies visits provided to visits ordered and scheduled using the clinical notes and the schedule.
- Notifies Clinical Team Coordinator of discrepancies between visits ordered, scheduled, documented.
- Coordinates external medical supply and DME requests with field teams, and verifies data systems are up to date with current patient information.
- Assists clinical team with ensuring patients and families have updated NOE addendum as requested.
- Builds, edits, and maintains assigned and ad-hoc reports and spreadsheets with accurate/up-to-date information and collaborates with appropriate staff as needed.
- Prepares schedules for weekend and on-call coverage.
- Collaborates with Office Support team to assist in providing good communication to patients, families, and vendors by participating in office call queue and as a back-up for other Office Support Team roles.
- Communicates with other medical professional, patients/families, and facilities to coordinate the sharing of information in compliance with HIPAA.
Qualifications
· High School Diploma required; some college preferred.
· Demonstrated experience in customer service and clerical work.
· Experienced in Microsoft Office products required.
· EMR experience preferred.

Job Type: Full-time
Pay: Up to $23.00 per hour
Expected hours: 40 per week
