What You’ll Do in This Role:

Are you a compassionate caregiver with a heart for helping others? As a Certified Nurse Aide (CNA) in our day center, you'll play a vital role in ensuring our participants receive the highest quality care. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact in the lives of those we serve.

Primary Responsibilities:

• Communicates effectively with participants, families, staff and the staff of other agencies
• Participates in Interdisciplinary Team (IDT) meetings as directed
• Provides basic ADL nursing care to participants
• Documents observations, nursing care and participant activities on appropriate forms and in the medical record as appropriate
• Reports changes in the participant’s condition to the pertinent IDT member
• Active participation in the Quality Improvement Process (QIP)
• Clean facility equipment and maintain clean facility condition
• Reports on participant status or family concerns to the DCS
• Completes home visits when directed
• Abides by all established PACE of Southwest Michigan policies, rules and regulations

✅ What You’ll Need:

➡️A current and valid Michigan CNA Certification (a must!)
➡️Experience caring for the elderly? Awesome! If not, we’ll teach you.
➡️The ability to read, write, understand, and follow directions like a pro.
➡️A heart for helping others and the physical ability to lift, stand, bend, transfer, stretch, walk, push, and pull as needed.
