Occupational Therapist

Accelerated Home Health Care is seeking a licensed and compassionate Occupational Therapist (OT) to provide in-home occupational therapy services to patients under physician orders and in coordination with the interdisciplinary care team.

Key Responsibilities:

• Provide in-home occupational therapy services , including Start of Care (SOC) and revisit visits.
• Complete patient evaluations and establish individualized treatment goals.
• Implement evidence-based interventions to improve independence in activities of daily living.
• Educate patients and caregivers on adaptive techniques, safety strategies, and use of equipment.
• Accurately document evaluations, treatment notes, and progress reports.
• Communicate patient status and progress with the care team.
• Follow physician orders, care plans, and all applicable state, federal, and agency regulations.

Minimum Certification & Licensing Requirements:

• Active Occupational Therapist (OT) license in the State of Michigan.
• Completion of required state and agency onboarding/credentialing.
• Up-to-date TB test.
• Valid driver’s license and reliable transportation.

Preferred Qualifications:

• Home health experience preferred (not required).
• Strong clinical judgment and communication skills.
• Ability to work independently in a home-based setting.
• Organized, reliable, and patient-focused.

Working Conditions:

• Services provided in patients’ homes ; travel between visits required.
• Flexible scheduling, including daytime hours and occasional weekends.
• Physical demands include standing, bending, lifting, and assisting patients as needed.
