Housing Case Manager
Salary: starting at $55,181.18 annually - placement above minimum salary is based on experience.

- Bachelor’s degree in social work, psychology, human services, or related field.
- One year experience working with adults diagnosed with an intellectual /developmental disability or mental health disorder.
- Community-based service delivery
- Knowledge of person-centered planning process
- One year of professional experience in homeless services
- Lived experiences with mental illness/developmental disabilities/co-occurring disorders/substance use disorders .
- Deliver services according to funding, regulatory, and best practice standards.
- Orient clients to housing programs for which they are eligible.
- Maintain working knowledge of community resources and available rental housing units.
- Develop and maintain effective working relationships with landlords and property managers.
- Actively engage clients in development and implementation of an individualized service plan
- Facilitate access to available financial subsidies and concrete supports.
- Internal and external case collaboration and coordination, including gathering and exchanging relevant information.
- Documentation of services and supports in accordance with funding, regulatory, accreditation and best practice standards.
- Support individuals on assigned caseload through advocacy, monitoring, referring, and linking with other providers.
- Participate in supervision, team, and other identified meetings.
- Perform other duties as assigned.
