What You'll Do In This Role:
Be a trusted helping hand in the moments that matter most. As a Home Health Concierge, you’ll provide personalized, non-clinical support to participants—often one-on-one—in their homes or in care settings such as assisted living or skilled nursing facilities. Guided by the interdisciplinary team, you’ll focus on enhancing comfort, independence, and overall well-being through meaningful, participant-centered support. Key Responsibilities
- Provide Hands-On Care – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.
- Support Mealtime– Help participants enjoy their meals and stay hydrated for good health.
- Keep Things Moving – Support mobility, including turning, positioning, and safe transfers.
- Perform Vital Checks – Measure and record vital signs, weight, and intake/output.
- Maintain Comfort & Care – Keep rooms tidy, cozy, and well-stocked with essentials.
- Provide Responsive Support – Answer call lights quickly and respond with kindness and compassion.
- Monitor with Watchful Eyes – Report any changes in conditions to the leadership ASAP.
- Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Skills, Knowledge & Expertise
- At least one year of experience working with frail elderly or long-term care populations. High School diploma or GED, required
- Reliable transportation and a valid Michigan Driver’s License.
- CPR certification or willing and able to obtain as offered by LifeCircles.
- Strong desire working within a collaborative, team-based model of care.
- Comfortable working independently and managing a self-directed workload.
