Home Health RN Job Details

• Position Title: Home Health Registered Nurse (RN) – Locum Tenens
• Location: Portage, MI 49002 and surrounding southern Kalamazoo County (including 49024, 49052, 49087, 49088, 49097; occasional support in northern St. Joseph County)
• Assignment Type: Travel/Contract (Locum Tenens)
• Start Date: August 25, 2025
• Assignment Length: 13 weeks
• Schedule: Monday – Friday, 8:00 AM – 5:00 PM (8-hour shifts, 40 hours/week)
• Estimated Weekly Salary Range: $2,270 - $2,570 per week

Registered Nurse Qualifications

• Minimum 1 year of recent Home Health RN experience (within the last 3 years) (required)
• Proficiency with HCHB (Homecare Homebase) electronic medical records (required)
• OASIS documentation experience (required)
• Active Michigan Registered Nurse license (required at time of submission)
• Current BLS certification (required)

Home Health RN Responsibilities

• Provide skilled nursing care to patients in their homes, ensuring compassionate and quality clinical support
• Perform comprehensive patient assessments and develop individualized care plans using OASIS documentation
• Collaborate with interdisciplinary healthcare team members to optimize patient outcomes
• Educate patients and families on disease management, medications, and self-care techniques
• Maintain accurate, timely documentation in HCHB (Homecare Homebase)
• Travel within assigned Michigan territory to deliver home health care and support neighboring counties as needed
