Home Health Speech Language Pathologist

· Requirements:

o Must have state Speech Language Pathologist License

o Must meet productivity expectation of 30(FT) or 15 (PT) points per week

o Must have at least 12 months of Home Health experience within the past 3 years

o HCHB and Oasis Experience Required

· Locations:

o Kettering, OH

o Troy, MI

· Pay Rate: $50/hr &$.50/mile

· Shift Details: Monday-Friday, 8hr shifts, 40 hrs a week, no weekends/holidays

· Job Description:

o Assessment and Diagnosis: Evaluating patients' needs, strengths, and weaknesses related to speech, language, voice, and swallowing.

o Treatment Planning: Developing individualized treatment plans that address specific functional needs, including goals, strategies, and interventions.

o Therapy Delivery: Providing skilled speech therapy services to improve communication and swallowing abilities, including techniques to improve speech production, language skills, and swallowing mechanics.

o Documentation: Maintaining accurate records of patient assessments, treatment plans, progress, and outcomes.

o Communication and Collaboration: Communicating with patients, families, and other healthcare professionals to coordinate care and ensure patient needs are met.

o Discharge Planning: Participating in care coordination activities and discharge planning to ensure patients have the necessary support and resources upon returning home.

o Professional Conduct: Maintaining the highest standards of professional conduct and confidentiality.

o Education and Training: Providing instruction and training to patients, families, and caregivers on how to implement therapy strategies and use assistive devices.

o Adapting Treatment: Adjusting treatment plans and goals as necessary based on patient progress and changing needs.
