Essential functions (job duties):
Job Purpose:
Acts as behavioral health clinician for Wayne State University Adult HIV/AIDS Program. Working within an interdisciplinary clinical setting performs basic psychosocial assessment, intakes, assists patients in resolving barriers to medical care. Serves as liaison for community agencies providing mental health and psychosocial services which support patient population. Reports to Psychosocial Team Leader.
Essential Functions:
Provides counseling, therapy and crisis intervention to patients regarding treatment, problems with depression, anxiety, substance abuse, domestic violence, grief and loss. Completes psychosocial assessments of People with HIV. Monitors the patient's psycho-social state and well-being across the continuum. Documents clinical visits in electronic medical records. Complies with established departmental policies and procedures and ethical standards.
Collaborates with other health team members (Physicians, Nurses, Patient Advocates, Medical Assistants, Case Managers) to develop a treatment plan appropriate for each client. Maintains detailed records of client referrals to community mental health treatment programs based on unique needs and circumstances. Follows up, tracks and reports on both internal and external psycho-social patient referrals. Attends pre-clinic meetings.
Coordinates patient appointment schedules for behavioral health and psychiatric clinical services. Assists psychiatrist with medication refills for patients. Completes patient's Behavioral Health and family medical leave paperwork. Handles patient correspondence, as necessary.
Collaborates and maintains positive relationships with external agencies and internal stakeholders that provide behavioral health or enabling services support to patient population. Educates patients on health insurance enrollment/benefits and options.
Develops monthly, quarterly or ad hoc program reports for leadership, quality team and external agencies. Supports quality management activities and projects.
Assists with the writing and development of grant proposals. Participates in program site reviews and audits.
Performs other related duties as assigned.
Unique duties:
Qualifications:
Education: Master's degree
Master's degree in social work from an accredited college or university. State of Michigan licensure and certification in social work is required.
Experience: Experienced (minimum 2 years of job-related experience)
Two years of clinical social work experience, preferably in a primary care, medical or public health setting. Experience working with people living with HIV and LGBTQ population preferred.
Knowledge, Skills and Abilities:

Knowledge of health information related to people living with HIV.

Ability to maintain cultural sensitivity in all actions.
Skilled in use of Electronic Health Records.
Ability to complete work and articulate provider concerns in a timely and concise manner.
Knowledge of appropriate billing and documentation as a social worker.
Preferred qualifications:
Licensed Master Level Social Worker with 1-2 years of Clinical Experience, providing counseling/therapy and facilitating support groups
