*SUMMARY: * The position will support the community in attaining and maintaining holistic wellness addressing physical, mental and spiritual wellness needs through health education programming, community outreach event planning, and implementation of chronic disease prevention strategies.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provide excellent customer service to clients, vendors, fellow employees, and represent LTBB Health Department in a professional, positive manner.
• Recognizes and is sensitive to the cultural dimension inherent in the practice of health education in Native American communities. Must be culturally competent and meet the health literacy and linguistic needs of target populations.
• Work with several grant programs within Community Health to support the grant deliverables that address community wellness.
• Assist with Native Way gym operation and programming and work with clients to gain access and orientation to the gym.
• Educate community members across the lifespan to adapt to healthy lifestyles that reduce cardiovascular risk factors by developing a wellness plan that focuses on physical activity and nutrition.
• Develop relationships with wellness resources in the community and coordinate wellness activities as appropriate. Examples include Yoga, Self Defense, Tai Chi, Zumba, cultural traditions and arts, etc.
• Educate the Community and share information about traditional healers, cultural coordinator, case management services, behavioral health, smoking cessation, or other community resources.
• Maintain accurate records of all outreach contacts and activities. Document activities as required and collect data as needed for reports and grants.
• Complete monthly, quarterly reports and other grant-specific reporting.
• Assist with community outreach events throughout the year, including Michigan Indian Family Olympics, Annual Health Fair, Winter Festival, and other community engagement opportunities as assigned.
• Coordinate and plan Community Wellness activities. Examples include Native Way, Warrior Trail, Elders lunch and cultural activities.
• Other duties as assigned within scope of the position.

EDUCATION AND EXPERIENCE: Bachelors degree in health or social work related field required, with three years of experience working in the human services field. Experience providing health focused community outreach preferred. Experience working with Tribal community is preferred.

*FUNDING: * This position is funded by Special Diabetes Program for Indians, Good Health and Wellness in Indian Country grant, and IHS funding and will continue as long as funding is available.
