Job Description

The Integrated Case Manager is an interdependent member of the patient-centered care team responsible for assessment, planning, facilitation, care coordination, evaluation, and advocacy for patients and families with complex health care needs. Key responsibilities include:

• Conduct comprehensive assessments of patients and families’ biomedical, psychological, social, and functional needs to evaluate impact on recovery.
• Develop personalized, patient-centered care plans aimed at optimizing the care experience.
• Engage patients and families through advocacy, ongoing communication, health education, and facilitation of available resources.
• Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to help patients overcome barriers to goal achievement.
• Provide counseling and interventions related to treatment decisions and end-of-life issues, including Advanced Care Planning.
• Ensure seamless, safe transitions between care settings and advocate for appropriate service delivery within health plan structures.
• Collaborate with the patient’s care team to co-manage complex medical and social needs and facilitate interdependent care conferences.
• Continuously evaluate patient response to care plans and modify as necessary.
• Participate in process improvement initiatives, including data collection, analysis, and follow-up interventions.
• Facilitate interventions in cases involving child abuse, elder abuse, domestic violence, institutional abuse, and sexual assault.
• Perform other duties as assigned to support department and organizational goals.
  
  

Critical Information / Qualifications

• Minimum 5 years of acute inpatient case management experience, including discharge planning (telephonic, utilization management, or therapy experience does NOT count).
• Strong computer skills with experience in Epic EMR.
• Current Michigan RN license or LMSW license.
• BLS certification through AHA.
• Nursing degree (BSN preferred for candidates wishing to convert at assignment end) or MSW.
• Ability to quickly learn and perform efficiently with minimal unit-based training.
• Typical case ratio: 1:21-25 (determined by individual patient risk factors).
• Experience managing complex patient populations and facilitating care transitions.
• Able to work rotating weekends; day shift.

Education / Licenses Needed

• Required: Nursing degree (RN) or Master of Social Work (MSW)
• Required: RN or LMSW license in Michigan
• Required: BLS certification through AHA
