Schedule: Full Time, exempt | minimum 40 hours per week | Monday - Friday | 8:00 am - 4:30 pm
Department: Participant Care | LifeCircles PACE | Muskegon and Holland, MI
The Clinic Pharmacist is a key member of the LifeCircles PACE team, delivering comprehensive, participant-centered clinical pharmacy services that optimize medication therapy and improve health outcomes for older adults. Working collaboratively with the Director of Clinical Services, Medical Director, Primary Care Providers, the Interdisciplinary Team (IDT), contracted pharmacies, hospitals, and post-acute care facilities, this role ensures safe, effective, and evidence-based medication management while preventing gaps in care during transitions. The Clinic Pharmacist provides expert clinical guidance to identify and resolve medication-related concerns, oversees medication utilization and pharmacy benefit management (PBM) initiatives, promotes compliance with Medicare Part D regulations, and champions cost-effective prescribing strategies. This position will work between the Holland and Muskegon locations.
Key Responsibilities
Participant Care
- Conduct participant assessments, perform home visits, evaluate medication therapy, and monitor treatment outcome to optimize safe, effective care.
- Partner with the Interdisciplinary Team to identify and resolve medication-related concerns, communicate participant changes, support care planning and transitions, and document interventions in the plan of care.
- Counsel participants, caregivers, and families on medication use, side effects, and adherence, maintain timely and accurate documentation, and deliver exceptional participant-centered service.
Comprehensive Medication Review
- Oversee comprehensive medication reviews, medication utilization initiatives, and Medicare Part D compliance through the development and implementation of policies, procedures, and best practices.
- Monitor prescribing patterns, evaluate medication use, implement evidence-based improvements, conduct audits, and support cost-effective, high-quality pharmaceutical care.
- Provide guidance to pharmacy support staff, stay current on clinical best practices, and assist with Part D bid preparation and other strategic pharmacy initiatives.
Other Duties, Skills, and Knowledge
- Provide oversight and guidance to the pharmacy technician while fostering strong teamwork and delivering exceptional service to participants and the interdisciplinary team.
- Demonstrate excellent communication, critical thinking, organization, and adaptability to meet the evolving needs of the geriatric population in a self-directed environment.
- Utilize electronic medical records proficiently while upholding LifeCircles' mission, values, and commitment to a positive, participant-centered culture.
- Work at both Holland and Muskegon locations.

Skills, Knowledge & Expertise- Registered Pharmacist with the State of Michigan.
- Controlled Substance license with the State of Michigan.
- Doctor of Pharmacy degree or equivalent experience with the evidence of advanced clinical training and knowledge in participant care and applied therapeutics, including therapeutic drug monitoring.
- PGY1 Pharmacy Practice Residency with a focus in ambulatory care or geriatrics preferred.
- Board Certification in Geriatric or Ambulatory Pharmacy preferred.
- Must meet a standardized set of competencies before working independently under collaborative practice.
- Must demonstrate knowledge of managed care and progressive pharmacy practice.
- Experience in leadership, with demonstrated skills desired.
