Due to recent growth we need to bring on some additional team members.

We are looking for kind, compassionate nurses to provide one-on-one skilled nursing care to a single client in their home. The role focuses on promoting the client’s health, safety, comfort, and independence while delivering personalized medical care. This position requires clinical expertise, and the ability to work autonomously with minimal direct supervision.

We are seeking Full Time, Part Time or Contingent.

Key Responsibilities

• Clinical Care Delivery
  • Perform skilled nursing tasks
  • Monitor vital signs, pain levels, and clinical status; recognize and report changes promptly.
  • Assist with activities of daily living as needed (bathing, grooming, toileting, feeding, mobility).
• Care Plan Management
  • Implement, update, and communication log.
  • Conduct regular reassessments and communicate findings to the case manager.
• Professional Conduct
  • Maintain client confidentiality (HIPAA).
  • Arrive on time, in proper uniform/scrubs, and remain alert throughout the shift.

Qualifications Education

• Active, RN or LPN license in the state of practice.

Experience

• Minimum 1 year of clinical nursing experience

Skills & Abilities

• Strong clinical assessment and critical-thinking skills.
• Excellent verbal and written communication.
• Ability to lift/transfer clients safely (up to 50–100 lbs with assistive devices).
• Valid driver’s license and reliable transportation (for travel to client home).

Personal Attributes

• Compassionate, patient, and culturally sensitive.
• Comfortable working in a private home with varying family dynamics, and variable schedules (including overnights, weekends, holidays).

Physical Demands

• Prolonged standing, walking, bending, and lifting.
• Ability to respond quickly in emergencies.
