All levels of experience are encouraged to apply and will train the right new graduate. + **Job Type:** Per Diem + **Pay:** $65-$80 per visit + **Location:** In and around Forest Hills + **Schedule:** Flexible! **What We Offer!** + **1099** and **W-2** options to fit your preferred work style + Competitive salary + Longstanding Success + Opportunities for professional growth and development + A professional, supportive and fun work environment fostered by a team of well-respected clinicians who take pride in the challenges of meaningful work + Focus on Patient Care - Quick EMR (No Oasis required) + Great work/life balance + Flexibility and Autonomy - see patients any 7 days of the week + Preference given to see patients in your primary geographic area **Responsibilities:** + Accept and treat patients within your designated geographic area + Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans + Develop a plan of care and treatment goals in consultation with the physician, patient, and family + Plan, prepare and implement customized therapy programs to restore function and promote independence + Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques + Provide timely and accurate documentation for each visit + Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning + Deliver care in compliance with agency policies and professional practice standards **Qualifications** + Physical Therapist Program Graduate + Licensed Physical Therapist in the state of New York or eligible. + Mandatory Certifications: First Aid, AED, & CPR Certifications. + Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance. + Ability to travel to patients' homes within the assigned geographic territory **Additional Information** We are proud to be an equal opportunity and affirmative action employer.
