Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will play a crucial role in providing high-quality patient care in various settings, including hospitals, nursing homes, assisted living facilities, and home care environments. As an LPN, you will be responsible for assisting with patient assessments, administering medications, and supporting the healthcare team in delivering exceptional care to patients of all ages.

Responsibilities

• Provide direct patient care under the supervision of registered nurses and physicians.
• Administer medications and treatments as prescribed, ensuring compliance with safety protocols.
• Conduct patient assessments, including vital signs monitoring and health history documentation.
• Assist in developing and implementing individualized care plans for patients.
• Perform basic nursing procedures such as IV insertion, catheterization, and wound care.
• Maintain accurate medical records and documentation in compliance with HIPAA regulations.
• Collaborate with healthcare team members to ensure continuity of care.
• Educate patients and their families about health management and disease prevention.
• Participate in discharge planning and follow-up care coordination.

Skills

• Medical office experience

• Assisted living knowledge
• IV infusion skills
• Nursing home experience
• Hospital experience (including emergency room and ICU)
• Triage capabilities
• Experience working with individuals with developmental disabilities
• Alzheimer's and memory care expertise
• Acute pain management knowledge
• Senior care familiarity
• Level I & II trauma center experience
• ICD coding (ICD-9 & ICD-10) proficiency
• Hospice and acute care experience
• Pediatric nursing skills
• Home care knowledge
• Phlebotomy expertise (venipuncture)
• Patient assessment abilities
• Basic life support certification
• Medical terminology understanding
• Experience administering injections
