- Resume (2 yr of LTAC experience).
- Skills checklist
- Basic Life Support - BLS Certification (must be from AHA- American Heart Association).
- 2 Supervisory Reference Forms, taken within the last two years
- Driver's license
- Covid Card
- Flu shot

- Pay rate up to $55/hr. (Taxable rate) and the Overtime Rate of $82.5/hr. respectively.
