Registered Nurse (RN) OR LPN – Home Health
- Payment is per visit
Position Summary:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our home health care team. The ideal candidate has experience in home health settings and is committed to providing high-quality, patient-centered care in the comfort of patients’ homes.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans
- Conduct comprehensive patient assessments and develop individualized care plans
- Monitor patient conditions and report changes to physicians and the care team
- Administer medications, treatments, and wound care as required
- Educate patients and their families on disease management, medications, and self-care techniques
- Maintain accurate and timely documentation in compliance with regulatory standards
- Collaborate with interdisciplinary team members to ensure coordinated care
Qualifications:
- Current and active Registered Nurse (RN) license (required)
- BLS (Basic Life Support) certification (required)
- Minimum of 1 year of home health experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Compassionate, patient-focused approach to care
- Ability to work independently and manage time effectively
- Valid driver’s license and reliable transportation
