Schedule: 2-3 days a week. 8 hour shifts

What You'll Do
As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:
- Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
- Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
- Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
- Guiding families through Advanced Directives with clarity and sensitivity
- Assisting with facility placement for patients when appropriate
- Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
- Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
- Participating in on-call duties on evenings and weekends as needed
- Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

Requirements:What You Bring
- Bachelor’s degree in Social Work
- Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
- Licensure and/or certification as required by the State.
- Minimum of 2+ years social service experience in a health care setting or social services agency.
- Experience in hospice, palliative care, or a related healthcare setting preferred
- Deep comfort with grief, loss, and end-of-life conversations
- Strong knowledge of community resources and care coordination
- Excellent documentation skills and attention to compliance
- Self-directed with the ability to work with little supervision
- Valid driver's license and willingness to travel locally for patient visits
