RN Case Manager

RN Case Managers work collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination. You will find various education opportunities within this role on top of getting to work for a top healthcare organization. **A caregiver in this position works days from 7:00AM - 7:30PM, including rotating weekends and holidays.** A caregiver who excels in this role will: + Ensure compliance with standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation. + Promote interprofessional synchronization throughout the acute episode. + Complete effective handoffs to the next level of care or community to ensure continuity of care. + Proactively assess, identify and work to mitigate the risk posed by barriers to patients' transitions of care. + Work with the patient and/or care partner(s) to develop a comprehensive, individualized transitional care plan. + Participate and lead in patient rounding activities, huddles and committee meetings. + Successfully complete annual competency training. Minimum qualifications for the ideal future caregiver include: + Graduate from an accredited school of professional nursing (ADN or BSN program) + Current state licensure as a Registered Nurse (RN) + Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross + Three years of recent RN clinical experience Preferred qualifications for the ideal future caregiver include: + Bachelor of Science in Nursing (BSN) + Certification by an approved entity within one year of eligibility (CTM, ACM or CCM through the American Heart Association) + Two years of recent experience in utilization review, quality or care management + Knowledge of acute care and accreditation requirements + Experience in discharge planning and quality improvement **Physical Requirements:** + Requires normal or corrected hearing and vision to normal range. + Ability to perform work in a stationary position for extended periods. + Ability to communicate and exchange accurate information + Ability to work with physical records, such as retrieving and filing. Ability to operate a computer and other office equipment. + Ability to travel throughout the hospital system. + Requires normal or corrected hearing and vision to normal range. + In some locations ability to lift up to 10 pounds **Personal Protective Equipment:** + Follows Standard Precautions using personal protective equipment as required for procedures **Pay Range** Minimum Annual Salary: $69,600.00 Maximum Annual Salary: $106,132.50 The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate's actual compensation will be determined after taking factors into consideration such as the candidate's work history, experience, skill set and education.
