We are seeking a dedicated and compassionate Home Care Physical Therapist to join University Home Care Inc’s team in Livonia, MI. In this role, you will provide personalized physical therapy services to patients in their homes, helping them recover mobility, reduce pain, and improve their overall quality of life. You will have the opportunity to make a meaningful impact by delivering one-on-one care tailored to each patient’s unique needs.

• Assess patients’ physical conditions and develop individualized treatment plans.
• Provide skilled physical therapy interventions in patients’ homes to improve mobility, reduce pain, and enhance function.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and their families about exercises, injury prevention, and therapeutic techniques.
• Maintain accurate and timely documentation of treatments and patient progress.
• Collaborate with healthcare professionals and caregivers to optimize patient outcomes.
• Adhere to professional standards, safety regulations, and company policies.

• Valid licensure as a Physical Therapist in the state of Michigan.
• Proven experience in physical therapy, preferably in home care or outpatient settings.
• Strong assessment and clinical reasoning skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation to travel to patient homes in the Southeast MI Region and surrounding areas.
• CPR certification preferred.

• Competitive pay at $70.00 - $80.00 per visit, paid bi-weekly.
