HIV LPN Navigator
OVERVIEW:
Under direction of the Medical Director, provides nursing services, supervises daily activities, and support staff for integrated HIV screening and PrEP (pre-exposure prophylactics) medications. Plans, organizes, and coordinates back-office operations to ensure efficient and effective clinical flow and utilization of resources for HIV screening and PrEP services. Works in collaboration with primary care providers, pharmacy staff, nursing staff, medical assistants, behavioral health staff, and community health staff.DUTIES, FUNCTIONS AND RESPONSIBILITIES:
- Utilizes nursing assessment protocols to accurately assess and triage HIV and PrEP patients
- Manages the point-of-care HIV testing and supplies for all Advantage clinics
- Works with providers and medical support staff to achieve efficient workflow
- Promotes and maintains a clinical environment that complies with federal, state, and local regulatory agencies
- Coordinates day-day activities including supervision, staffing, and problem solving
- Coordinates the provider availability with the patient appointment schedules
- Monitors workflow to ensure efficient and effective productivity
- Monitors patient satisfaction and responds to patients’ complaints in a timely manner
- Attends required meetings and participates in committees and in-service trainings as requested
- Prepares reports and manages assigned projects
- Assures compliance with HIPAA guidelines
- Accurately prepares and supervises the compilation of all month end closing procedures and reports
- Coordinates all ancillary services i.e., laboratory, pharmacy, and specialty clinics
- Provides patient care and services including vaccinations, walk-in injections, etc. as needed
- Always maintains a professional demeanor and effective communications
- Performs other related duties as required
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of the principles and practice of nursing
- Interest in working with HIV-positive or at-risk patients
- Knowledge of the state and federal laws pertaining to health-care delivery and to federally qualified community health centers
- Knowledge of the structure and function of community health centers
- Skill in communicating effectively with the staff, patients, and their families
- Skill in establishing and maintaining effective working relationships with staff, supervisors, and colleagues
- Ability to multitask and solve problems while maintaining a calm demeanor
- Proven leadership ability to effectively direct staff and the ability to follow directives from others
MINIMUM QUALIFICATIONS:
- Graduation from a Licensed Practical Nursing program or within one semester from graduation
- A minimum of two years progressively responsible experience in a medical office or equivalent clinical setting
- Excellent interpersonal, oral, and written communication skills
- Computer literacy and experience with an automated patient management system
LICENSES AND CERTIFICATIONS REQUIRED:
- Must be able to be licensed as a practical nurse in the state of Michigan and/or completed a certified License Practical Nurse program
- Must possess a valid Michigan driver’s license and BCLS certification
OTHER REQUIREMENTS:
- Must provide own vehicle for local travel.
