  • Focuses on preserving and enhancing the psychosocial well-being of residents.
  • May serve as primary backup for the admissions role in the absence of the RN Case Manager/Admissions Coordinator.
  • Provides psychosocial assessment at periodic intervals, as often as needed to include admission, quarterly, annual, and significant changes as applicable.
  • Creates an individualized care plan for all residents.
  • Welcomes new residents, provides support, and initiates a trusting relationship.
  • Contributes as an integral member of the interdisciplinary team, making specific recommendations to assist in the residents’ overall care and genuine well-being.
  • Provides “active listening” with residents who express a need to talk with a supportive individual for various reasons.
  • Assists in providing/overseeing behavioral interventions and monitors effectiveness.
  • Facilitates resident council meetings on a rotational basis with the Diversional Therapy department.
  • Facilitates discharge planning with residents who are at the facility for short-term rehabilitation, including initiation of written discharge plans, making referrals to appropriate agencies/resources, and ordering any durable medical equipment needed to facilitate a safe transition to home.
  • Monitors psychoactive medications of all residents.
  • Participates in various meetings and committees within the facility, including (but not limited to) Admissions meetings, QAPI, discharge meetings, Harmony Restoration meetings, stand-up, care conferences, comfort care meetings, and ethics.
  • Participates in in-services.
  • Complies with all state, federal, and local guidelines.
  • Complies with policies and procedures of the facility, as well as assist in the drafting of policies and procedures for the Social Services Department as needed.
  • Acts as a liaison between residents, families, the community, and the facility.
  • Coordinates/facilitates comfort care meetings with resident/family and interdisciplinary team.
  • Completes advance directives with residents/families upon admission, yearly, and on an as-needed basis.
  • Assists families/residents in completing Health Care Durable Power of Attorney paperwork as needed/requested.
  • Facilitates the activation and reactivation of Healthcare durable Power or Attorney paperwork as needed.
  • Initiates guardianships and attends court proceedings as needed.
  • Facilitates room changes within the facility.
  • Completes various resident assessments/evaluations, including (but not limited to) cognitive screens, depression screens, trauma screens, biopsychosocial assessments, elopement risk assessments, MDS assessments, and pre-admission screens in conjunction with OBRA.
  • Assists residents in obtaining needed items such as clothing or personal care items.
  • Assist residents in voting in elections as requested.

Education & Training:

Must possess a minimum of Bachelor of Social Work Degree (BSW) from an accredited institution and maintain an active license to practice social work in the state of Michigan. Two years of experience in long-term care preferred.

  • Must be able to perform all essential functions of the position.
  • Must work effectively both independently and as part of a multidisciplinary team.
  • Must be in good physical and mental condition and demonstrate a genuine willingness to work with older adults.
  • Must be able to demonstrate the skills, sensitivity, and professional judgment needed to understand and support residents, families, staff, and community partners.
  • Must be proficient in the use of computers and standard office equipment, including fax machines, copiers, and related technology.
  • Must be able to prepare clear, concise written reports and effectively communicate residents’ social and psychological needs.
  • Must demonstrate patience, tact, professionalism, a positive attitude, and the ability to interact appropriately with residents at varying levels of cognitive and emotional functioning.
  • Must be able to manage confidential information with discretion and in accordance with professional and regulatory standards.
