Licensed Practical Nurse / Registered Nurse - LTC

We are seeking a Licensed Practical Nurse (LPN) or Registered Nurse to join our healthcare team. The ideal candidate will provide high-quality patient care in a long-term care facility. As an LPN/RN, you will deliver essential nursing services while ensuring compliance with healthcare regulations and standards.

Setting : Short/long-term care, skilled nursing and rehabilitation facility.

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers.
• Conduct patient assessments and monitor vital signs.
• Assist with daily living activities for patients, including personal hygiene and mobility.
• Collaborate with healthcare team members to develop and implement patient care plans.
• Maintain accurate medical records and documentation in compliance with HIPAA regulations.
• Provide education to patients and families regarding health management and disease prevention.
• Participate in discharge planning and follow-up care coordination.
• Ensure a safe and clean environment for patients by adhering to infection control protocols.

Experience
Candidates should possess the following skills and experience:

• Experience in critical care, long-term care, assisted living, or home care settings is preferred.
• Familiarity with EMR/EHR systems such as Point-Click-Care
• Experience working with diverse populations including individuals with developmental disabilities, Alzheimer’s care, dementia care, or geriatrics.
• Proficiency in patient assessment techniques, medication administration, phlebotomy, and basic life support (BLS).
