Job Title: Certified Occupational Therapist (Home Health)
Job Location: Oakland, MI
Job Type: Contract
Job Description:
- Perform patient evaluations and develop individualized treatment plans
- Provide therapy to improve ADLs, mobility, strength, coordination, and cognitive skills
- Conduct home safety assessments and recommend adaptive equipment
- Educate patients and caregivers on therapy techniques and safety practices
- Monitor patient progress and update treatment plans as needed
- Maintain accurate documentation and collaborate with interdisciplinary care teams
Requirement:
- Graduate of an accredited Occupational Therapy program
- Current Occupational Therapist license in the state of practice
- NBCOT certification (if required by state)
- Current CPR certification
- Minimum 1 year of Occupational Therapy experience (Home Health preferred)
- Experience with patient assessments, treatment planning, and documentation
- Strong communication and time management skills
- Valid driver's license and reliable transportation
- Home Health experience
- Knowledge of Medicare and insurance documentation requirements
