* Experienced Gastroenterology Physician Assistant, Fulltime * (Holland)
CURRENT HOLLAND HOSPITAL EMPLOYEES- Please apply through Find Jobs from your Workday employee account.Provides health care services to patients under the oversight of a physician. Follows established standards, procedures, protocols, and practices, and gives specific patient care directions to nursing and other staff. This position requires the ability to assess, diagnosis, treat, care for, understand and react effectively to the unique needs of the patient population for whom services are provided. This position must be able to demonstrate the knowledge and skills necessary to provide care and service to patients of the ages cared for in their practice setting.
PATIENT CARE
- Under the limited direction of the attending physician, obtains and records patient medical history; performs physical examinations; orders, evaluates and monitors medical and laboratory diagnostic tests and treatment plans.
- Prepares, develops, and initiates treatment plan in consultation with attending physician; performs ultrasound guided injections and therapeutic procedures.
- Orders, evaluates, and monitors medical and laboratory diagnostic tests and treatment plans including laboratory tests, x-rays, pharmaceuticals, and interdisciplinary consults in collaboration with the attending physician or designee and according to established guidelines.
- Educates and counsels patient and family members on condition, desired results of treatment plans, expectations for recovery and other related areas.
- Conducts patient rounds in both the outpatient clinic and within the hospital and documents assessment, diagnosis, and treatment plan in progress note. Prepares discharge summaries, as assigned.
- Share in office call coverage.
EDUCATION/DISCHARGE PLANNING
- Determines patient/family educational needs and provides education to meet the identified learning needs.
- Facilitates discharge planning along with the patient, family, and other members of the healthcare team.
- Documents discharge plan and education provided.
- Participates in patient care planning discussions with patient, family, and other members of healthcare team when applicable in collaboration with physician.
QUALITY IMPROVEMENT/RISK MANAGEMENT
- Ensures provision of high quality of care.
- Participates in problem identification and continuous quality improvement related to clinical services and coordination of care as well as program/ staff growth and development.
- Maintains current knowledge related to practice excellence in specialty area.
- Attends educational programs, completes mandatory educational requirements, and reads appropriate professional literature to maintain and enhance skills and clinical competence.
- Routinely consults with appropriate administrative staff about incidents that potentially impact risk management to determine an appropriate action plan.
ADMINISTRATIVE
- Performs duties in accordance with medical staff bylaws, policies and rules and regulations of Holland Hospital Medical Group and the standards of accrediting agencies such as DNV, third party payers, as well as federal, state, and local government laws, rules, and regulations.
- Maintains records, charts, report, and forms in conformity with standards and policies. Assures documentation meets all requirements for a complete and accurate medical record.
