Are you looking for a rewarding position as a Private Duty RN or LPN? We are seeking a compassionate, dynamic, energetic, organized, and efficient professional with experience to join our Private Duty Family at our Charlevoix location , serving the surrounding areas.

For this role, you need to be a registered nurse or licensed practical nurse who sees the value of client care .

Requirements

• Current Michigan License in good standing
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient’s status and needs on a full time on call basis

Job Responsibilities:

• Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Document all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record.
• Supervision of paraprofessionals providing services to patient
• Maintain high level of customer satisfaction with our clients, families and caregivers
