Home Health Physical Therapist Assistant (PRN)

As a PRN Home Health Physical Therapist Assistant at AdvisaCare, you will work closely with patients in their homes, providing assistance with their treatment plans under the direction of a licensed Physical Therapist. You will be an essential part of our team, helping to support the care of individuals and families within the community in which you live!
Responsibilities
- Implement physical therapy treatment plans as directed by a licensed Physical Therapist
- Prepare and maintain accurate and timely clinical documentation
- Assist patients with exercises and activities to promote strength, safety, and mobility
- Provide education to patients and their families about their conditions and treatment plans
- Ensure a safe and effective home environment for clients
- Collaborate with interdisciplinary team members, including nurses, physical therapists and occupational therapists, to coordinate patient care
Requirements
- Current state licensure in Michigan as a Physical Therapist Assistant
- Minimum of one year of experience in home health care is preferred, but not required
- Strong communication and interpersonal skills
- Ability to work independently and manage multiple patients effectively
- Valid driver's license and reliable transportation
