Chronic Care Management (CCM) Specialist

Job Summary
We are seeking a compassionate and dedicated Care Specialist to join our team. In this role, you will provide essential support and care to patients in need, ensuring their well-being and enhancing their quality of life. Responsibilities
- Conduct comprehensive assessments of patients with chronic conditions, including gathering medical history, understanding the patient’s social determinants of health, and identifying care needs.
- Monitor patient health status through regular phone calls or texting and report any changes or concerns to the appropriate healthcare professionals.
- Maintain accurate records of patient care and progress.
- Participate in ongoing training and professional development opportunities.
- Educate patients on disease management, lifestyle modifications, self-monitoring, and medication adherence to empower them to take an active role in managing their health.
- Maintain accurate and up-to-date documentation in the electronic health record (EHR) system.
- Track patient outcomes and progress towards care goals and document any changes or updates to care plans.
Qualifications
- Associate’s or Bachelor’s degree in Public Health, or a related field.
- Previous experience in a caregiving role is highly desirable.
- Strong communication skills, both verbal and written.
- Ability to work independently as well as part of a team.
- Compassionate nature with a strong commitment to providing high-quality care.
- Flexibility to work various shifts, including evenings and weekends as needed.
- Excellent organizational and time management abilities.
