We are seeking a compassionate and dedicated Care Specialist to join our team. Responsibilities

• Conduct comprehensive assessments of patients with chronic conditions, including gathering medical history, understanding the patient’s social determinants of health, and identifying care needs.
• Monitor patient health status through regular phone calls or texting and report any changes or concerns to the appropriate healthcare professionals.
• Maintain accurate records of patient care and progress.
• Participate in ongoing training and professional development opportunities.
• Educate patients on disease management, lifestyle modifications, self-monitoring, and medication adherence to empower them to take an active role in managing their health.
• Maintain accurate and up-to-date documentation in the electronic health record (EHR) system.
• Track patient outcomes and progress towards care goals and document any changes or updates to care plans.

Qualifications

• Associate’s or Bachelor’s degree in Public Health, or a related field.
• Previous experience in a caregiving role is highly desirable.
• Strong communication skills, both verbal and written.
• Ability to work independently as well as part of a team.
• Compassionate nature with a strong commitment to providing high-quality care.
• Flexibility to work various shifts, including evenings and weekends as needed.
• Excellent organizational and time management abilities.
