Job Description:

We are seeking a compassionate and experienced Family Nurse Practitioner (FNP) to provide primary care services in Detroit. This short-term contract role focuses on adult and geriatric populations, emphasizing preventive care, chronic disease management, and patient education in a collaborative healthcare environment.

Responsibilities:

• Provide comprehensive primary care to adult and geriatric patients
• Conduct patient assessments, diagnoses, and treatment planning
• Manage acute and chronic conditions, including pain management
• Prescribe medications and monitor treatment effectiveness
• Educate patients on disease prevention and health promotion
• Collaborate with multidisciplinary healthcare teams
• Maintain accurate documentation and utilize medical software systems

Requirements:

• Active Nurse Practitioner (NP) license – Michigan (required)
• Minimum 2+ years experience in:
• Patient assessment
• Chronic disease management
• Patient education
• Medication management/prescribing
• Team collaboration
• Strong clinical and communication skills
