Pomeroy Living is looking for a compassionate and motivated Social Worker who wants to make a real impact in the lives of older adults. In this role, you will help residents and their families navigate important life transitions while ensuring each resident receives the support, dignity, and advocacy they deserve.

What You'll Do

• Conduct psychosocial assessments to identify resident needs and concerns
• Support residents and families with counseling, adjustment to care, and emotional support
• Participate in interdisciplinary care plan meetings
• Advocate for resident rights, dignity, and quality of life
• Assist with discharge planning and coordination of community resources
• Work closely with nurses, therapists, physicians, and other team members
• Lead resident and family meetings when needed
• Maintain documentation in accordance with state and federal guidelines
• Provide guidance to staff regarding psychosocial concerns

What We're Looking For

• Bachelor's Degree in Social Work or related human services field such as sociology, gerontology, psychology, rehabilitation counseling, or special education
• One year of supervised social work experience working directly with individuals in long term care or rehabilitation
• Michigan Social Work license preferred
• Strong communication and interpersonal skills
• Ability to think critically and solve problems
• Someone who genuinely cares about improving the lives of older adults
