Direct Care Supervisor (DCS)

Job Summary

The Direct Care Supervisor provides day-to-day oversight of direct care staff delivering hands-on support to clients/residents. This role ensures services are delivered safely, ethically, and in compliance with individualized care plans, agency policies, and applicable state and federal regulations. The Supervisor serves as the primary link between management and frontline staff, supporting quality care, staff performance, and regulatory readiness.

Essential Duties & Responsibilities

Staff Supervision & Support

  • Supervise, mentor, and support Direct Care Workers (DCWs), caregivers, and aides during assigned shifts.
  • Conduct shift oversight, task delegation, and real-time problem resolution.
  • Monitor staff performance and provide coaching, corrective guidance, and documentation as needed.
  • Ensure adequate staffing levels and report shortages or call-offs to management.

Care Delivery & Client Safety

  • Ensure care is delivered in accordance with each client’s service plan, IPOS, or care plan.
  • Observe client conditions and report changes in physical, emotional, or behavioral status.
  • Respond to incidents, emergencies, and behavioral escalations per agency protocol.
  • Ensure proper use of universal precautions, infection control, and safety procedures.

Documentation & Compliance

  • Review and verify staff documentation (daily notes, progress notes, MARs, incident reports).
  • Ensure documentation is accurate, timely, and compliant with payer and regulatory standards.
  • Participate in internal audits, licensing reviews, and survey preparation.
  • Report suspected abuse, neglect, or exploitation in accordance with mandatory reporting laws.

Training & Quality Assurance

  • Assist with onboarding, orientation, and on-the-job training of new staff.
  • Reinforce required trainings (recipient rights, infection control, emergency procedures).
  • Participate in quality improvement activities and corrective action plans.

Communication & Coordination

  • Serve as point of contact between direct care staff and management.
  • Communicate professionally with clients, families, case managers, and interdisciplinary team members.
  • Participate in team meetings and contribute to care coordination efforts.

Required Qualifications

  • High school diploma or GED (minimum).
  • Minimum 1–3 years of direct care experience in home care, residential care, AFC, or similar setting.
  • Demonstrated leadership or lead-staff experience.
  • Ability to read, write, and document in English clearly and accurately.
  • CPR/First Aid certification (or ability to obtain).
  • Ability to pass background checks as required by state law.

Preferred Qualifications

  • CNA, CMA, QMAP, or equivalent credential (state-dependent).
  • Supervisory experience in Medicaid-funded or licensed care settings.
  • Familiarity with person-centered planning and behavior support plans.
  • Knowledge of state licensing rules (e.g., AFC, group home, adult day).

Physical & Work Requirements

  • Ability to lift, transfer, and assist clients as required by the care setting.
  • Ability to stand, walk, and respond to emergencies during shifts.
  • Flexibility to work evenings, nights, weekends, or on-call as required.
