Home Health Visits with Growth Potential
Job Summary:We are looking for a compassionate Speech-Language Pathologist (SLP) to provide home health and outpatient therapy services. This role offers opportunities to deliver Remote Therapeutic Monitoring (RTM) and maximize therapeutic impact for patients within Medicare’s guidelines. Key Responsibilities:
- Perform speech, language, and swallowing assessments in home health settings.
- Develop personalized therapy plans to address communication and swallowing disorders.
- Deliver RTM services, including device management and progress monitoring.
- Educate patients and families on therapy techniques for effective communication.
- Document all care and services accurately to ensure compliance.

Remote Therapeutic Monitoring (RTM):
- Utilize RTM technology to monitor and manage patient progress remotely.
- Ensure compliance with RTM CPT codes and Medicare billing requirements.
Outpatient Therapy Services:
- Provide therapy services in a clinic setting or through telehealth.
- Deliver personalized treatment plans for musculoskeletal and other conditions.
Requirements for RTM and Outpatient Therapy:
- Willingness to obtain a Medicare Provider ID.
- Services will be billed under a group using the group’s Provider ID.
