We are seeking a compassionate and dedicated Licensed Medical Social Worker to join our home health team. In this vital role, you will provide comprehensive psychosocial assessments, emotional support, and counseling services to patients and their families in a home-based setting. Responsibilities

• Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social support systems.
• Develop individualized care plans that incorporate behavioral health strategies, crisis intervention, and motivational interviewing techniques.
• Provide individual, family, and group therapy sessions focused on mental health counseling, addiction counseling, dementia care, PTSD care, and working with individuals with developmental disabilities or autism.
• Collaborate with healthcare teams to coordinate post-acute care plans, hospice services, or assisted living arrangements while ensuring compliance with HIPAA regulations and accurate medical documentation.
• Assist patients with navigating community resources, housing options, and support services to promote independence and well-being.
• Maintain detailed medical records using Electronic Medical Record (EMR) systems, ensuring accuracy in documentation of diagnoses such as ICD-10 codes and CPT coding for billing purposes.
• Participate in clinical research initiatives related to behavioral health or home care best practices to continually enhance service quality.

QUALIFICATIONS

1. Active LMSW License

2. Social work license in the State of Michigan

3. Master's degree from a Council on Social Work Education-accredited institution

4. At least 2 years of experience in Home Health/Hospice
