About Grand Valley Medical Specialists

Grand Valley Medical Specialists (GVMS) is an independent, physician-owned primary care practice dedicated to providing high-quality, patient-centered care. We believe exceptional healthcare starts with strong relationships, preventive care, effective chronic disease management, and a commitment to continually improving the health of our patients.

As healthcare continues to evolve, we're focused on strengthening our quality programs and preparing for the future of value-based care. We're looking for a Clinical Quality Manager who is passionate about improving patient outcomes, using data to drive meaningful change, and partnering with providers and staff to deliver the highest standard of care.

Position Summary

The Clinical Quality Manager will oversee the practice's quality improvement initiatives and payer quality programs. Working closely with physicians, advanced practice providers, clinical staff, care coordinators, and leadership, this role will identify opportunities to improve clinical outcomes, close care gaps, strengthen quality performance, and support value-based reimbursement initiatives.

This position is ideal for someone who enjoys analyzing data, solving problems, building relationships, and turning quality metrics into practical improvements that benefit both patients and providers.

Key Responsibilities

· Monitor performance across Medicare, Medicare Advantage, Medicaid, and commercial payer quality programs.

· Review quality metrics, payer scorecards, and population health reports to identify trends and opportunities for improvement.

· Develop and implement strategies to improve quality measures, patient outcomes, and payer performance.

· Partner with providers and clinical staff to support accurate documentation, coding, and evidence-based care.

· Identify and help close preventive care and chronic disease management gaps through workflow improvements and patient outreach.

· Work with care management, clinical staff, front office staff, and providers to improve care coordination and patient engagement.

· Create reports and dashboards that provide meaningful performance data for physicians and leadership.

· Stay current on quality reporting requirements, payer incentive programs, and value-based care initiatives, including HEDIS, MIPS, Medicare Star Ratings, ACO, PCMH, PGIP and PIP requirements.

· Coordinate quality improvement projects and monitor progress toward established goals.

· Develop workflows, educational resources, and staff training to support quality initiatives.

· Build collaborative relationships with payer representatives, physician organizations, and other external partners.

· Present quality performance results, recommendations, and progress updates to practice leadership.

Qualifications

Required

· Three or more years of experience in healthcare quality improvement, clinical performance, population health, or value-based care.

· Experience working in a primary care or physician practice setting.

· Experience analyzing healthcare quality data and implementing performance improvement initiatives.

· Working knowledge of HEDIS, Medicare Star Ratings, MIPS, preventive care measures, and value-based reimbursement programs.

· Experience with Electronic Health Record (EHR) systems and population health tools.

· Strong analytical, organizational, and project management skills.

· Excellent communication and relationship-building skills.

· Proficiency with Microsoft Office, including Excel, Word, and PowerPoint.

Benefits

· Competitive salary based on experience

· Health, dental, and vision insurance

· Paid time off and paid holidays

· Retirement plan with employer contribution
