Salary:
Harbortown Treatment Center, a substance abuse treatment center located in Benton Harbor, MI, is currently seeking a Nurse Practitioner to join our organization. The Nurse Practitioner will assist in care of individuals admitted into the program and will provide necessary follow-up care while on Medication Assisted Treatment medications. The mid-level practitioner will work on the coordination of care for additional patient medical issues, as needed.
Essential Duties and Responsibilities:
- The NP will work with the Medical Director, Physician, Physician Assistant, and other medical providers for medical management of the clinics patients;
- Ensures that evidence of current physiological dependence, length of history of addiction, or exceptions to criteria for admission are documented in the patient's record before the patient receives the initial dose;
- Ensures that a medical evaluation including a Medical history has been taken, and a physical examination has been done before the patient receives the initial dose;
- Ensures that appropriate laboratory studies have been performed and reviewed;
- Signs or countersign all medical orders as required by Federal or State law. Medication order changes (dosage adjustments);
- Performs annual history and physicals;
- Communicates pertinent patient information to the Medical Director;
- Educate the person served about their diagnosis;
- Documents all patient visits appropriately in the electronic medical record (EMR);
- Maintains patient confidentiality per HIPPA requirements and (42 CFR Part 2);
- Learns all medical protocols and procedures used by the clinic and ensures compliance throughout the multidisciplinary team;
- Develops and collaborates on medical treatment plans based on evidence-based practices and evaluates outcomes;
- Communicated and collaborates with physicians, nurses, and other members of the team to resolve patient care problems and enhance care delivery; and
- Keeps current on new knowledge gained from conferences, workshops, professional literature, or other training and assimilates this knowledge into clinical practice.
Qualifications:
- Active Michigan Nurse Practitioner license
- DEA certificate, or on the process of obtaining certificate
- Meets credentialing requirements where assigned, as well as the ability to be credentialed with Medicare, Medicaid, and commercial insurance payers.
