Remote RN Case Manager – 13-Week Contract | $42/Hour (Paid Weekly)

Location: Remote (Must be licensed in Michigan)
Schedule: Full-Time (40 hours/week) | Day Shift
Pay: $42/hour | Weekly pay
Contract Length: 13 weeks
About the Role:

Our hospital client is seeking a compassionate and experienced RN Case Manager to join our team on a contract basis . In this remote role, you will coordinate patient care, manage case workflows, and ensure optimal health outcomes—all while working from home. If you have a strong background in case management and a passion for patient advocacy, we’d love to hear from you!
Key Responsibilities:

✔️ Oversee and coordinate patient care plans in collaboration with healthcare providers
✔️ Conduct virtual assessments, monitor progress, and ensure continuity of care
✔️ Provide education and support to patients and families regarding treatment plans
✔️ Utilize clinical expertise to manage transitions of care and discharge planning
✔️ Maintain accurate documentation within electronic health records (EHR)
✔️ Work efficiently within a remote team setting while adhering to compliance standards
Requirements:

🔹 Active RN license in Michigan (required)
🔹 Minimum 2 years of case management experience (preferred)
🔹 Strong communication and decision-making skills
🔹 Familiarity with EHR systems and remote patient coordination
🔹 Ability to work independently and meet performance metrics

Bachelor’s degree in nursing or related professional field (i.e., social work, counseling, health education, etc.) or a Master’s degree of Social Work, required.
Minimum (3) three years of clinical experience, required. 
Registered Nurse (RN) or a Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license, required.
Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association.
