What You’ll Do
- Develop individual care plans with input from the individual and their family, based on the individual’s personal preferences and choices.
- Assist individuals and families in coordinating, selecting, and overseeing their own support.
- Work collaboratively as part of a multidisciplinary team to ensure timely integration of services.
- Develop and implement behavioral guidelines and interventions; track and analyze related data.
- Participate in training that promotes sensitivity to cultural and socioeconomic diversity.
- Meet face-to-face with assigned individuals as medically necessary per their Individual Plan of Service.
- Provide consultation to direct care staff regarding clinical and program-related matters, as applicable.
- Provide individual and group therapy sessions, as assigned; assist in developing and facilitating group sessions.
Work Schedule
Standard Hours: Monday-Friday, 8:00am-5:00pm, on-site
Flexible scheduling available based on caseload and program needs
On-Call Rotation: Participation required; Rotates one week on, one week off
On-Call Pay Enhancements
- Weekdays (Mon-Fri): $50 per on-call shift
- Weekends (Sat-Sun): $100 per on-call shift
- Holidays: $150 per on-call shift
Qualifications
Minimum Education: Bachelor’s degree in Social Work, Psychology, or a related Human Services field required; Master’s degree in Social Work or Psychology preferred.
One or more of the following credentials strongly preferred:
- Licensed Bachelor Social Worker (LBSW) or Limited Licensed Bachelor Social Worker (LLBSW)
- Licensed Master Social Worker (LMSW) or Limited Licensed Master Social Worker (LLMSW)
- Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), or Temporary Limited Licensed Psychologist (TLLP)
- Social Service Technician (SST) Certification
Preferred Experience:
- 1–2 years of experience working with individuals with developmental disabilities (DD) or mental illness (MI).
- Prior knowledge of the community mental health system and/or Medicaid-funded services.
- Experience working with electronic medical records (EMR).
