Nurse Practitioner
Senior Community Care, a PACE (Program of All-Inclusive Care for the Elderly) provider, is seeking a Nurse Practitioner to deliver primary and preventive care to seniors in home, clinic, and community-based settings. You will conduct assessments, develop and manage care plans, prescribe medications, and collaborate closely with an interdisciplinary team of physicians, nurses, therapists, and social workers. This role is ideal for a compassionate, team-oriented clinician who values dignity, respect, and holistic care and wants to help older adults remain safely independent in their communities.
