We are seeking a dedicated Licensed Practical Nurse (LPN) specialized in transport and 1:1 school-based care to join our dynamic healthcare team. In this vital role, you will provide comprehensive nursing services to students with complex medical needs during transportation and within the school environment. Your expertise will ensure safe, efficient, and compassionate care for students requiring individualized attention, contributing significantly to their health and educational success. The ideal candidate will possess a strong foundation in patient assessment, medical documentation, and emergency response, with a commitment to promoting health and safety in a school setting.

Locations

Seeking on-call sub for transport to school 1:1, via bus or private van. Meet at bus garage or student's home... ride to school and back to your vehicle.. split shift / AM route and PM route.. times vary depending on student's school. High school starts earlier than Elementary school

Several openings, Full-time / Part-time / PRN-Subs within Wayne & Oakland County School Districts. Transport only is also available. Immediate start needed as well.

Students cannot attend school without a nurse, medically fragile students that require 1:1 nurse staff.

Duties

• Administer medications, including injections and IV infusions, following prescribed protocols while maintaining strict adherence to HIPAA regulations and medical documentation standards.
• Monitor vital signs and conduct physical examinations to assess student health status before, during, and after transportation or school activities.
• Provide direct bedside care for students with specialized needs such as tube feeding, catheter care, or ventilator management.
• Assist with emergency situations by managing airway obstructions, performing basic life support (BLS), and coordinating urgent medical interventions as needed.
• Collaborate with multidisciplinary teams to develop and implement individualized healthcare plans tailored to each student's requirements.
• Ensure safe transportation by preparing students for travel, securing medical equipment, and responding promptly to any health concerns during transit.
• Conduct ongoing assessments of student condition, adjusting care plans accordingly while ensuring compliance with all regulatory standards.
• Educate students, families, and school staff on health management strategies such as medication administration, infection control practices, and emergency procedures.
• Support discharge planning by coordinating follow-up care and communicating effectively with healthcare providers.

Experience

• Trach Care / Emergency Reinsertion
• Changing HME (trach nose), as needed
• 1:1 Ventilator Patient
• Suctioning - Trach, nasal, oral
• G-tube - Feeding, Problem Solving, Care of tube and site
• Asthma - Nebulizer, inhaler
• Seizures - Monitor/Recognize/Treat/Medicate
• MUST be able to lift 40lbs
• Experience in XSI setting preferred

Also :

• RELIABLE AND PUNCTUAL IS A MUST
• Pediatric Patient Care experience is a must
• Should possess good organizational, verbal and writing skills to communicate with the staff and parents.
• Be Willing to Work as a Team with Teachers, Parents, Nurses
• Be Licensed in State of Michigan
