Join our team of ACT Team Members to Work with the client and his/her chosen support network in developing treatment and recovery plans that promote individual choice, the use of community resources, and formal and informal supports in pursuit of the individual’s desired future. Assists the client in the assignment of available agency resources to the plan and monitor those supports and services according to the plan. Services and supports are based on the principles of recovery and self-determination. Your office is surrounded by nature, located in Benzie and Manistee Counties, just miles from beaches and hiking trails. Benzie and Manistee are a short drive to Frankfort, Traverse City, Cadillac, and Ludington, charming towns outfitted with shopping, unique restaurants and breweries.

About the Role
The Case Manager works with adults living with Severe Mental Illness (SMI) and adults living with Intellectual/Developmental Disabilities (I/DD), and individuals living with co-occurring substance use disorder. Works with the client and his/her chosen support network in developing treatment and recovery plans that promote individual choice, the use of community resources, and formal and informal supports in pursuit of the individual’s desired future. Assists the client in the assignment of available agency resources to the plan and monitor those supports and services according to the plan. Services and support are based on the principles of recovery and self-determination.
Your role is to:
- Coordinates quality person centered planning for clients and ensures its implementation based on the principles of recovery and trauma informed care. Ensures the implementation of self-determination principles that incorporates strengths and abilities.
- Provides outreach service to clients in their homes and in the community.
- According to person/family centered plans provides advocacy, linking, coordinating, and monitoring to assist with housing, education, transportation, employment, financial, social, psychiatric, and other clinical and community inclusive activities.
- Monitors mental and physical health status, including monitoring medication use and management.
- Provides appropriate interventions designed to stabilize and improve functioning and assists the clients to experience a positive recovery process from a severe mental illness and/or co-occurring disorder.
- Assists the clients to achieve quality of life.
- Provides assessments and crisis stabilization of clients experiencing a mental health crisis including consultation with primary care providers, other organizations, and individuals.
- Provides substance use disorder treatment interventions to clients with co-occurring disorders.
- Coordinates skills training and support in daily living skills and in activities designed to foster recovery.
- Makes referrals and facilitates access to health care and community resources and assists with discharge planning for assigned clients.
- Provides timely clinical documentation in the electronic medical record that includes amount, scope and duration of services provided in accordance with the person-centered plan.
- Promotes recovery and respects the choices and autonomy of clients while teaching and encouraging actions that will promote good mental and physical health.
- Collect, record and summarize data on observable client behavior.
- Collaborate with the treatment team including clients, parents and caregivers, outside professionals and co-workers.
- Perform duties in accordance with well-defined policies, procedures and instruction.

Requirements:Qualifications and Requirements
- Bachelor’s degree in Social Work or a Bachelor’s Degree in a Human Service Field required as outlined by the State of MI Medicaid Provider Manual.
- At least 18 years of age.
- Valid Michigan’s driver’s license and good driving record.
- Ability to sustain State Licensure in field of practice and/or Certifications to perform duties of the job.
- Ability to be certified in First Aid and CPR within 90 days of hire (agency available training)
- Ability to participate in agency provided training in the principles of behavior, behavioral measurement and data collection, functions of behaviors, basic concepts of ABA, generalization and its importance in sustainability of learned/acquired skills and medical conditions/illness that impact behaviors.
Preferred
- Full Michigan licensure preferred or Michigan Limited License in field of practice considered.

Work Environment:
CWN prioritizes a work/life balance. Your supervisor will work with you to utilize flex time and paid time off. You will primarily work in the office with the following schedule:
- Monday through Friday
- Between 8:00am and 5:00pm
You may be required to work hours outside of the regular schedule if assigned by your supervisor to meet clients’ needs.
