- Assignment Type: Travel RN Case Manager (Locum Tenens)
- Start Date: August 26, 2025
- End Date: November 22, 2025
- Duration: 13-week contract
- Schedule: 8-hour day shifts (8:00am – 4:30pm), rotating weekends
- Hours per Week: 40 (5x8 hour shifts)
- Guaranteed Hours: 40 per week
- Case Ratio: 1:21-25 (based on patient risk factors)
- Estimated Weekly Salary: $2000-$2050 per week
- Minimum 5 years of recent acute inpatient case management experience (within the last year)
- Strong computer skills; proficiency with Epic EMR required
- BLS certification through AHA
- Current, valid Michigan RN license or LMSW with current valid Michigan licensure
- Associate or Bachelor’s degree in Nursing, or Master’s in Social Work (MSW)
- Experience in acute discharge planning
- Excellent verbal and written communication skills
- Strong problem-solving, analytical, and decision-making abilities
- Familiarity with community resources and managed care preferred
- Ability to adapt quickly and work efficiently with minimal training
- Conduct comprehensive assessments of patient and family/caregiver needs (biomedical, psychological, social, and functional)
- Develop and implement individualized, patient-centered care plans
- Engage patients and families as part of the care team through advocacy, communication, and health education
- Utilize critical thinking, motivational interviewing, and self-management techniques to address patient barriers
- Provide counseling and interventions for treatment decisions and end-of-life care, including advanced care planning
- Coordinate safe and seamless transitions between care settings
- Advocate for appropriate healthcare services within the patient’s health plan
- Collaborate with interdisciplinary teams to co-manage patients with complex needs
- Participate in process improvement initiatives to enhance patient outcomes and reduce risk
- Facilitate interventions in cases involving abuse or neglect
- Support departmental and organizational healthcare goals
