In this position, you’ll play a central role in patient-centered care. Your responsibilities include comprehensive psychosocial assessments, discharge planning, and risk assessment to ensure safe transitions from hospital to home or alternative care settings. You’ll coordinate with interdisciplinary teams to align treatment plans with patient goals, identify and address barriers to care, and connect patients and families with housing, transportation, financial assistance, caregiver support, and community resources. You’ll guide families through complex decisions, advocate for patient rights, and help patients navigate insurance, palliative and hospice options, and caregiver needs. While many assignments provide rewarding of support and growth, there is no guaranteed minimum number of hours, so the schedule can adapt to patient flows and facility requirements.
