Overview of Position:
Under the direction of the Manager of Behavioral Health with the established policies and procedures, the Integrated Care Behavioral Health Clinician promotes efficient coordination of patient care through collaborative practice within the health center for all populations served. This includes, but is not limited to, brief social work behavioral interventions, short term behavioral health and SUD counseling, assessments and referrals (internal and external) to the appropriate level of care. Responsible for working with the providers, nurses and all team members, and referring out to community resources to assist in addressing personal environmental and socioeconomic issues that inhibit positive health outcomes.
Minimum Requirements:
Master’s Degree in Social Work from a CSWE accredited program AND Full licensure in the State of Michigan as Licensed Master’s Social Worker (LMSW), Clinical preferred. Will consider a candidate with a Limited License as a Masters Level Social Worker (LLMSW) credential. The employee with a Limited License must take and pass the ASWB Examination within six (6) years in accordance with the State of Michigan requirements to retain employment. Note: GHS will follow all state of Michigan requirements. If the requirements change, the job description will be updated to reflect such changes. All licensed clinical staff must maintain licensure as a condition of employment. Minimum of two years’ experience in community-based health care setting; medical social work experience preferred

- Vision – Position requires the ability to visually assess details at both near and far distances, as well as to perceive depth and spatial relationships with or without reasonable accommodations.
- Hearing – Position requires the ability to receive and respond to verbal communication with or without reasonable accommodations.
- Lifting, Standing, and Walking With or Without Reasonable Accommodations:
- Position requires the ability to lift up to 35lbs on an occasional basis
- Position requires the ability to regularly stand for extended periods of time
- Position requires the ability to walk at a minimum of 35% up to a maximum of 65%
- Position will require the ability to occasionally stoop, bend, or reach to perform work tasks with or without reasonable accommodation
- Position must be able to perform tasks requiring manual dexterity, including the use of hands and fingers for handling, grasping, typing, writing, or operating equipment with or without reasonable accommodation.
Additional Requirements:
Substance use disorder credentials or immediate readiness to obtain such credentials.
Valid Michigan Driver’s license and use of a vehicle during working hours.
Must be able to meet the physical and sensory demands of the position.
Veterans must submit Form DD214 to determine eligibility for Veterans’ Preference
