CHW - ACT Program
One (1) year of experience providing community-based support in BH, social services, housing, or related field. The CHW - ACT is a core, in-field member of the multidisciplinary ACT team who delivers intensive, recovery-oriented, person-centered services in the community, home, and other natural settings. Under ACT team supervision, this role provides basic services and support essential to maintaining beneficiaries’ ability to function in community settings—including assistance accessing food, housing, medical/behavioral health care, benefits, and social/vocational activities—consistent with the Michigan Medicaid Provider Manual and Michigan ACT Field Guide fidelity specifications.
Education & Experience
- High school diploma or GED required.
- Michigan Community Health Worker certification required.
- Basic Life Support (BLS) certification required.
- MI Medicaid CHW registry required.
- Valid Michigan Chauffeur’s Driver’s License is required
Preferred Qualifications
- Knowledgeable about National C3 Core Competencies.
- Peer support experience/lived experience as a consumer/family member within BH.
- Experience working with co-occurring SUD, SMI, homelessness, and justice-involved populations.
